No-cost home-care available for eligible seniorsBy: Staff Report
The California Health Collaborative’s (CHC) Multi-Purpose Senior Services Program (MSSP), funded by the California State Department of Aging, is accepting new enrollments. MSSP provides no-cost social and health care management services for elderly persons, who are eligible for Medi-Cal and certifiable for nursing facility care, to ensure these individuals live safely at home for as long as possible.
MSSP services include a talented team of nurses, social workers and care aides who make telephone and/or home contact with every client on a monthly basis to ensure they are getting the care they need to live safely at home.
Eligible clients must be 65 years of age or older, live within Sacramento, Placer or Yolo County, be able to be served within MSSP’s cost limits, be currently eligible for Medi-Cal, be certified or certifiable for placement in a nursing facility, and need assistance with three or more daily living tasks such as dressing, bathing, grooming and toileting. MSSP may also be able to provide additional services such as adult day care, housing assistance, chore and personal care assistance, respite, transportation, etc., based on funding and health and safety assessments.
